Job Description

The Advanced Care Practice Manager is responsible for the overall management of the clinical program. This position oversees and manages the daily clinical and nursing practice for programs within the scope of the division, creates treatment plans, provides patient assessment, medical triage, and care coordination within the scope of APP licensure. This is primarily an administrative position, but direct patient care is required within the scope of the position. This position provides direct management of the clinical and administrative staff, oversees clinical competence, and provides oversight for staff performance, skills development, and human resources related items.

The position is responsible for maintaining and creating collaborative internal and external relationships with referring entities, physicians, insurance companies, and vendors in order to provide effective and efficient care and outstanding patient and client experience. This position fosters positive and professional interpersonal relationships and promotes a diverse environment that is respectful of all cultures and backgrounds.

This position is accountable for developing, conducting, and analyzing quality and process improvement initiatives; planning,
delivering, and evaluating clinical programs; and establishing, measuring, and achieving program metrics. Additional responsibilities may include project oversight of clinical support programs, and developing policies and procedures.


This role serves as a professional role model, leader, mentor, and clinical resource/educator for members of the team. This position could report directly to Medical Director and/or Director of Advanced Practice Providers, with a dotted line to other roles as applicable. Any supervision of nursing staff will report through the Chief Nursing Officer.

Qualifications:
Minimum Education
Master's Degree as an Advanced Practice Nurse from an accredited University and eligibility for medical staff credentialing (Required)

Minimum Work Experience
3 years minimum work experience as an Advanced Practice Provider
Experience leading quality improvement projects
Experience leading teams and/or managing staff in a healthcare setting (Required)

Required Skills/Knowledge
Strong knowledge of clinical specialty
Strong knowledge of pediatric outpatient and inpatient clinical specialty related to the population served by the department
Strong leadership skills with the ability to consider multiple variables to determine proper course of action
Excellent written and verbal communication skills that are adaptable for family, staff, and leadership levels
Demonstrates the ability to work independently and/or with a team managing multiple and conflicting priorities
High proficiency with MS Office and other computer applications as required
Outstanding customer service and service recovery skills
Ability to manage multiple projects and high volume of work with a positive attitude

Required Licenses and Certifications
Advanced Practice Registered Nurse (APRN) Licensure in the state(s) of clinical practice
CDS and DEA in state(s) of clinical practice.
BLS certified. (Required)

Functional Accountabilities
Clinical
  • Develops plan of care for patients including specialist selection, identifying necessary procedures, and creating cost estimates, as applicable.
  • Communicates with families and patients throughout the patient care experience to understand their goals and needs for medical treatment and ensure a well-coordinated visit.
  • Assess, diagnose, monitor, and promote clinical status of each patient utilizing evidenced based practice and medical knowledge.
  • Writes patient care orders, prescribes therapy, and provides necessary home care orders as needed.
  • Identifies significant changes in patient status using physical exam, lab values, diagnostic tests, and patient/family input.
  • Differentiates between normal and abnormal findings.
  • Performs procedures as needed after obtaining appropriate credentialing.
  • Participates in and leads multidisciplinary patient care and planning conferences.
  • Is a clinical resource for patient, parents and the interdisciplinary team.
  • Provides on-call supervisory oversight for the clinical care of patients in conjunction with the Medical Director, as applicable.
  • Facilitate interdisciplinary patient care rounds/conferences to review treatment goals and optimize outcomes.
  • Collaborates with nursing, medicine, and other members of the healthcare team to regularly ensure quality patient care.
  • Leads daily clinical operations and partners with team members, care providers, payers, as needed to support timely, efficient, cost effective optimal care.
  • Supervises and coordinates clinical care of patients served by the division.
Patient & Client Relationships
  • Establish, build and sustain effective relationships with business contacts for the purpose of providing the best client experience to the division's internal and external contacts.
  • Works to improve patient and client satisfaction
  • Resolves conflicts related to the division's processes with patients/families, external agencies, and payers, and elevates concerns to the division's leadership or appropriate parties as needed.
  • Plan, consult and collaborate with patients, families physicians, nurse practitioners, physician assistants, and other professional colleagues; involve patients and families in shared decision-making; advocate for patients and families; access relevant health services.
  • Utilize expertise in cultural competency to enhance relationships, processes, and outcomes for patients/families.
  • Communicate and escalate clinical/administrative issues as appropriate to CNH levels of individuals assigned for resolution.
Human Resources Management
  • Participates in the day-to-day management of staff.
  • Participates in the interview, recruitment, and hiring of new staff.
  • Completes annual performance appraisals for direct reports.
  • Oversees departmental orientation and completion of competency for newly hired team members.
  • Oversees work distribution to department members to assure volumes are adequately distributed and the care environment is running smoothly.
  • Manage conflict effectively by giving constructive and objective feedback.
Performance Improvement/Outcomes Management
  • Establishes processes to evaluate delivery of care and services with related tools for measurement of outcomes.
  • Participates, coordinates, and/or represents Children's National for individual and group educational presentations within the organization and outreach events focused on quality improvement, health education, and other programs as designated.
  • Provides suggestions for system enhancement to improve patient care services offered by the division.
  • Tracks and reports inpatient, outpatient, outreach, and ancillary care delivery metrics identified by the program.
  • Responsible for quality management through appropriate utilization and management of education and resources.
  • Reviews and analyzes quality reports, identifying key indicator trends and developing plans to ensure best practices are implemented.
  • Evaluates care coordination of the division's patient population to identify opportunities for quality improvement.
  • Employ key attributes of effective collaboration to create, promote, and maintain an environment that supports successful partnership and high-performance team outcomes in concert with departmental and hospital-wide goals/objectives.
  • Contributes to the sustainability and advancement of nursing excellence which translates into ongoing Magnet designation.
  • Participate in and support shared nursing leadership.
